  • Looking to rent a Eugene School District 4J facility? Submit your request online through Facilitron, and expect approval within 5-7 business days (submit at least two weeks in advance). All users must provide proof of insurance, sign a contract, and pay fees before their event—payment is conveniently processed through the system using credit cards, checks, or PayPal. Limited summer availability runs from July to mid-August. Weekly recurring rentals are often possible depending on school activities. Remember to include all setup, rehearsal, and takedown time in your reservation, and note that school supplies, equipment, and inflatable games are not permitted.   • Concussions

     

     

  • All students participating in school-based extracurricular sports in grades 6-12 must have a physical examination on file before they can participate. Physical exams are required once every two years for continued participation.

    This requirement applies to:

    • Grades 7-12 (per Oregon state law ORS 336.479)
    • Grade 6 (per Eugene School District 4J policy)

    To submit your student's physical examination, download and complete the official form from the Oregon Board of Education, then upload both pages (1 and 2) of the signed form to FinalForms before practice or competition begins. The athletic administration will then update your student's records with the new expiration date.
