4J extends medical, vision and dental benefit offerings to Licensed Substitutes who meet eligibility criteria outlined in the Collective Bargaining Agreement between EAST and 4J. Eligibility is reviewed annually and a notification of eligibility is sent via district email in August. If you have been notified that you are benefit eligible, you MUST complete enrollment even if you are waiving benefits!

4. Enroll in benefits for the 2023-2024 Plan year:
   MyOEBB Online enrollment portal closes on September 15th, 2023
   2023-24 Guest Teachers Benefits Enrollment Form– If you are unable to complete enrollment online, please return a completed enrollment form to Human Resources no later than September 7th, 2023
5. ALL Licensed Substitutes enrolling in benefits MUST return an ACH Form for Benefit Premium Payment to HR by September 7th, 2023
