Overnight/High Risk Field Trip Process
Follow the steps below to complete the Overnight/High Risk Field Trip process.

- Download the Itinerary Form to your computer.
- Open the Itinerary Form, complete it, and save it.
- You will need this Itinerary Form in Step 2.

- Click the link below to begin the Overnight/High Risk Field Trip Form.
- Towards the bottom of this form, you will be asked to upload the Itinerary you completed in Step 1.
- Once you submit this form, check your email for a confimation message containing your Trip ID. Save your Trip ID – you will need it in Step 4.

- Download, complete, and save to your computer any required documents below.
- No more uploading documents to a Google Drive folder! You can now upload your field trip documents directly in Step 4.
- You must be signed in to your 4J Google Account to access these forms.

- Download, complete, and save to your computer any required documents below.

- After you have completed and saved your documents, find your school below and click the link.
- Enter your Trip ID from the confirmation email described in Step 2, and upload your completed documents.

What Makes A Field Trip High Risk?
Field Trip Risk Assessment
Examples of High Risk Activities that may require extra documentation, equipment, and research for authorization. Each trip or activity will be considered for approval on an individual basis.
These include, but are not limited to:
● Camping
● Recreational/Commercial watercraft (This requires a documented recent
inspection report. If the event is on a river, a test run should be done before the
actual event.)
● High ropes courses (approval depends on circumstance and location)
● Tree climbing (approval depends on circumstance and location)
● Beachcombing (approval depends on circumstance and location)
● Destination located on or in proximity to lakes or rivers
● Snow tubing/skiing/snowboarding (approval depends on circumstance and
location)
● Archery (requires certification and dependent upon location)
● Swimming in pools (requires a certified and insured lifeguard. Certification and
insurance reviews are required before approval)
● Hiking (All students should have a whistle, compass, and map on their person
and be instructed on using them should they become lost.)
● Mountain climbing, rock climbing, or rappelling
● Horseback riding
● Hang gliding
● Snow camping
● Noncommercial airline air travel
● Ziplines
Activities that are STRICTLY PROHIBITED include:
● Swimming in lakes and/or rivers
● Spelunking (approval depends on circumstance and location)
● Skydiving
● Trampolines, inflatable playground equipment, or bounce houses
● Events where an adult is alone with one child (other than parent or guardian)
● White water rafting or watercraft (use of watercraft in excess of 30 feet, contact
your district supervisor for exceptions in unique circumstances)
● Aircraft - operation of an aircraft
● Hang gliding
● Bungee jumping
● Bonfires
● Fireworks
● Water parks
● Haunted houses
