Dependent Life Insurance

 

Eugene School District 4J offers benefit eligible employees the option of purchasing optional life insurance coverage for their spouse or domestic partner, and their eligible dependent children. OEBB rules require each eligible employee to purchase supplemental life insurance for themselves in an amount equal to or greater than any coverage purchased for a family member. The cost of this optional coverage is found on the Rates page.

For Spouse and Domestic Partner Optional Life Insurance coverage you can elect coverage up to $500,000. Coverage may be subject to medical underwriting approval if you enroll outside of the initial enrollment period or if the requested amount is greater than the guaranteed issue amount. Newly benefit eligible employees can enroll their spouse or domestic partner into (up to) the $30,000 guaranteed optional life insurance amount. Enrollment into any amount grater than the $30,000 up to the $500,000 maximum amount will need a medical history statement form to be completed and submited. Insurance for the additional amount may be denied on the basis of that information, but the guaranteed amount will still be approved. 

For Optional Child(ren) Life Insurance coverage you can elect up to $10,000. Remember, in order to elect the optional child(ren) life insurance coverage, you must also elect Employee Supplemental Life coverage.
