For Mac OS users, there is a system setting that may not allow you to tab onto several types of elements in a web page. To change this setting:
Eugene School District 4J offers benefit eligible employees the option of purchasing additional life insurance for themselves, to supplement the basic coverage provided by the district.
You can elect coverage up to $500,000. Coverage may be subject to medical underwriting approval if you enroll outside of your initial enrollment period or if the requested amount is greater than the guaranteed issue amount.
Newly benefit eligible employees can enroll in up to the $200,000 guaranteed Optional Employee Life Insurance coverage amount. Enrollment into any amount greater than the $200,000 up to the $500,000 maximum amount will need a medical history statement form to be completed and submited. Insurance for the additional amount may be denied on the basis of that information, but the guaranteed amount will still be approved.
You can enroll or change your life insurance elections as a New Hire, during Open Enrollment or within 31 days of a qualifying status change (QSC). The guaranteed issue will only be available for new employees or newly added dependents. Other QSC changes will require medical history statement form to be completed and submitted in a timely manner. Insurance may be denied on the basis of that information.
