Licensed New Hire Documents
Please ensure to review the instructions thoroughly before filling out your paperwork. If you do not have access to a printer, you can visit our HR/Finance Resource Center and they will print the paperwork for you. Some new employees choose to come early on orientation day in order to complete their paperwork. You can reach the Resource Center at 541-790-7670.
Please note, if you do not have your onboarding packet completed before orientation, you will be asked to reschedule for the next week’s class.
All of the following is required for new hire orientation:

- Employment Eligibility Verification (I-9)
- Equal Employment Opportunity Form
- Federal W-4
- The Human Resources Department is unable to give tax advice regarding the W-4 form. If you have questions about your withholding please consult with your accountant prior to your processing appointment.
- Oregon State W-4
- Direct Deposit
- Child Abuse & Sexual Conduct Disclosure Release
- Verification of Previous Work Experience
- This form also serves as your Sick Leave Balance Transfer form. Any sick time accrued at your previous employer can be transferred to 4J.
- Licensed Professional Salary Placement
- District Policy and the District Policy Signature Page
- Payment Options for Fingerprint Processing
- This form is for non-TSPC Licensed positions only. If you have your TSPC license, you do not need to get fingerprinted again.
- Driving Record Authorization Form
- This is only required if you will be driving a district vehicle.
Fingerprinting Process
All employees who are not licensed and fingerprinted by the Teacher Standards and Practices Commission (TSPC) are required to be fingerprinted by the Oregon Department of Education (ODE) in order to be employed with Eugene 4J. If you are licensed with TSPC OR if you have worked for 4J/another Oregon school district in the past AND have not moved out of the state since then, you are already covered and can skip this step.
Please note, you should not book an appointment until directed to by your HR representative.
The total cost for fingerprinting is $78.50, broken out below. The process is quick and easy:
- Schedule an appointment with our third-party provider, Fieldprint Oregon. There are several Eugene/Springfield locations for your convienence. There is a $12.50 appointment fee that is paid when you book the appointment.
- After you attend your appointment, Fieldprint Oregon will send your prints to the ODE for your background check. Once this is complete, you are clear to attend new hire orientation.

Next Steps
Now that you have your fingerprints scheduled and your packet filled out and ready for orientation, you can start getting familiar with a few of our systems and softwares that will assist in the day-to-day of your new position.
- Activating your 4J E-Mail Account
- You will not be able to activate your 4J email until you have completed orientation and are entered into the HR/Payroll system by your representative. Once you receive your employee ID number then you will be able to activate your 4J email account.
- Vector Safe Schools
- This is our annual safety training provider. You will need to complete a series of courses at the start of your employment. Your username will always be your employee ID. Please make a note of this for yourself if needed. While you have outstanding safety courses to complete, you will receive weekly reminder emails titled “Vector Training, K-12 Edition” from “Eugene School District 4J” with a “safeschools.com” domain which includes your username and a link to the login page.
- Frontline Absence Management
- Frontline is how you will submit requests for time off. You can find guides on how to get started here on our Frontline home page.
- Synergy (Student Information System)
- If you will be working directly with our students, you will likely need an account with our Student Information System. Your supervisor will confirm for you during your first days on the job if you’ll need access.

- Mandatory Reporter Information
- Working for a public school district means you are a mandatory reporter. This flier explains what that means as well as what to look out for during your day-to-day.
- Standards of Performance for 4J Teachers
- These are the legal standards for all our educators and will guide any performance reviews conducted during your employment.
- Salary Schedules
- Salaries are negotiated with EEA, the licensed employee union that operates within 4J. HR is unable to negotiate with individuals, as pay rates are based upon an employees’ prior experience history and education.
- Work Year Calendars
- These calendars give details on work days and paid holidays for each type of employee.
- EEA Bargaining Agreement
- This is the original Agreement between Eugene 4J and EEA. It details all your rights, benefits, and restrictions as a licensed employee. It is highly advised to review and understand this Agreement.
