Supplemental Student Accident Coverage

How to Purchase Coverage
For cost and ordering information, request a brochure called Student Accident & Sickness Insurance. The brochure and insurance information are available through our Risk Management deparment or your school office. Claim forms are available from Risk Management Services at (541) 790-7537.

School-Time Plan

Student Coverage for Preschool through 12th Grade.
Interscholastic Sports (except tackle football) for Preschool through 8th Grade.

Covers accidents that occur at school during the school day and one hour before or after school while the student:

• Is on the school premises.
• Attending a school-sponsored and supervised activity.
• During travel between home and school to attend regularly scheduled classes or to participate in school activities.
• In school-provided transportation vehicles anytime.

Does not cover:

• Accidents that occur during interscholastic sports events for students in grades nine through 12.
• Accidents in certain types of motor vehicle accidents (being struck by two- and three-wheeled vehicles).

24-Hour Plan

Student Coverage for Preschool through 12th Grade.
Interscholastic Sports (except tackle football) for Preschool through 8th Grade.

Covers children 24 hours a day from the first day of the school year until the first day of the following school year including all:

• Weekends.
• Summer vacations.
• Other vacations such as Winter and Spring Break.

Does not cover:

• Accidents that occur during interscholastic sports events for students in grades nine through 12.
• Accidents in certain types of motor vehicle accidents (being struck by two- and three-wheeled vehicles).

24-Hour Dental Accident Plan

For Preschool through 12th grade students This coverage is also available to teachers, administrators and other district personnel.

The 24-Hour Dental Accident Plan is designed to cover treatment for accident-related injuries to teeth.

Covers reasonable and customary charges by dentists in this area for:
• Injuries incurred 24 hours a day during all activities and all forms of transportation.
• Dental injuries for which treatment is sought within 30 days of injury.
• Usual and customary treatment charges for up to one year of the accident date.
• Specific coverages include:

-All necessary examinations.
-X-rays.
-Repair of existing caps and crowns.
-New dentures and bridges.
-Endodontia related to the accident.

Does not cover:
Payment until all amounts you receive from other insurance or health agreements have been paid.
• Treatment for complications or re-injury of an existing condition.
• Treatment for infections, except those occurring through an accidental open wound.
• Orthodontics.
• Loss or damage to dentures or braces.

Interscholastic/Tackle Football Plan

For Ninth-12th grade Interscholastic Sports and Tackle Football

Covers:

• Interscholastic tackle football season for students in grade nine-12.
• All interscholastic sports for students in grades nine-12.
• All practice sessions, official games and related travel from the first day of official practice (or when payment for coverage is received) until the last day of the season.
• Travel to and from practice and games.
• From the first day of official practice (or when payment for coverage is received) until the last day of football season.

Does not cover:

• Certain conditions and injuries. These are described in the “Student Accident and Health Insurance” brochure available in the school office.
