School Nurses

4J school nurses are assigned to several schools and rotate to provide services. School nurses can help students and families access health care at the health centers or with community providers, including helping to access vision and dental care for uninsured students; set up care plans for students who have health problems, offer consultation services to the school sites and to families.

School-Based Health Centers—Open to All 4J Students

4J health centers4J has two school-based health centers to serve the district’s elementary, middle and high school students. The health centers, located at Churchill and North Eugene high schools, are open part-time during the school year and closed during school holidays.

  • Both health centers are staffed with a nurse practitioner, a 4J health assistant, a medical assistant and mental health therapists.
  • The health centers can handle most health issues, including illnesses, injuries, ongoing health problems, well-child physicals, reproductive health services, and immunizations.
  • The centers also offer students mental health counseling in partnership with Lane County Behavioral Health for depression, anxiety, stress, anger, and attention and focusing issues, and can give referrals for substance abuse treatment.
  • The health centers can bill private insurance and the Oregon Health Plan and are committed to making sure our patients get the care they need, whether they can afford to pay or not. Please call the clinics for hours and to schedule appointments.
